Serenity Oaks welcomes elders and adults with disabilities throughout the Twin Cities. We accept private pay as well as the following Minnesota waiver programs:
CADI Waiver
Community Access for Disability Inclusion — for adults under 65 with disabilities.
DD Waiver
Developmental Disabilities Waiver — long-term services and supports.
Elderly Waiver (EW)
Home and community-based services for those 65+ eligible for Medical Assistance.

Reach out
Call us or submit a contact form. We'll listen, answer questions, and schedule an in-person tour.

Tour & meet the team
Visit the home, meet our caregivers, and explore whether Serenity Oaks is the right fit.

Care assessment
We complete a care assessment with the resident, family, and case manager (when applicable).

Care plan & paperwork
We build a personalized care plan and coordinate with case managers on waiver paperwork.

Move-in day
We welcome your loved one home — gently, warmly, and with a plan in place from day one.
