Resident rights
Every resident at Serenity Oaks is entitled to a clear set of protections. We post these rights in the home, review them at admission, and revisit them whenever a service plan changes. A complete written copy is provided in our Resident Handbook.
To be treated with courtesy, respect, and recognition of personal dignity.
To privacy in personal care, medical treatment, communication, and visits.
To participate in care planning and to make informed choices about treatment.
To a written service plan that reflects current needs, preferences, and goals.
To accept or refuse services, including medication, after being informed of risks.
To voice concerns or grievances without fear of retaliation, and to receive a timely response.
To manage personal finances and to keep and use personal possessions when safe.
To receive visitors and communicate privately with family, friends, and advisors.
To advance written notice before any change in services, rates, or relocation.
To a safe, clean, and homelike environment with reasonable accommodations.
Serenity Oaks operates as a licensed Assisted Living facility in the State of Minnesota. Our license, inspection history, and complaint records are publicly available through the Minnesota Department of Health.
Every resident receives a written assessment, a personalized service plan, a uniform disclosure of services and fees, and an Arbitration Agreement that is explained in plain language at admission.
Residents and families may raise concerns directly with our Director of Care, or contact the Minnesota Office of Ombudsman for Long-Term Care for free, independent advocacy and support.
